Requirements:

REMINDER:

• Please print/xerox all the documents to A4 size.
• No staple allowed (to all requirements).

BASIC REQUIREMENTS: (FILIPINO PASSPORT)

(First Time to apply Japan Visa)

1. Duly accomplished and signed APPLICATION FORM (A4 Size)
2. Original Passport – 6 months valid prior to departure date.
3. 2pcs. Colored Picture with  white background, 4.5cm x 4.5cm.
4. Latest Income Tax Return – with clear photo copy
5. Original Bank Certificate
6. NSO Authenticated Birth Certificate (issued within a year).
7. NSO Marriage Contract (issued within a year)
8. Travel Itinerary – (draft of day to day visit/activity in Japan)

FOR EMPLOYEE:

• Original Employment Certificate
• Approved Leave of Absence

FOR BUSINESSMAN:

• Copy of Business Permit from SEC or DTI
• Copy of Business Permit of Mayor’s Permit

FOR STUDENTS/CHILD:

• Original Affidavit of Support of Parents/Guardian – notarized (If traveling relatives or friends)
• If traveling with parents no need
• Photocopy of School I.D.

FOR RETIREE:

• Photocopy of Senior Citizen Card or Retirement Documents
• Original Copy of Marriage Contract (if traveling with spouse)
• Affidavit of Support if Senior is dependent

HAS PREVIOUS JAPAN VISA:

• Attached the visa/passport no need to submit NSO Birth Certificate

NOTE:

ADDITIONAL REQUIREMENTS:

1. Exemption of Birth Certificate and Marriage Contract (if the applicant has his/her passport (old or valid) with a used Japanese Visa).
2. If Birth Certificate from NSO is unreadable, or if there is no record in NSO, together with “Certificate of Non-Record” from NSO Headquarter, submit:
   • Birth Certificate from Local Civil Registrar
3. If Birth Certificate is LATE REGISTRATION(NSO), submit:
   • Baptismal Certificate
   • School Record (form 137)
   • School Yearbook (if applicable)

FOR PROC & ROC PASSPORT

Above Requirements must submit all the documents,

1. I-Card (xerox only)

FOR ROC WITH TAIWAN I.D.

***NO NEED TO APPLY VISA

Processing Time:

5-7 working days

**NO PERSONAL APPEARANCE REQUIRED.

IMPORTANT REMINDERS:

• Please submit complete documents to avoid delays in processing.
• Please do not leave any blank in the application form. Kindly put “N.A.” if it’s not applicable.
• Make sure that the back page of the passport is completely filled-up (address, tel numbers and name of person to be contacted in case of emergency.)
